it says "add friend", and then his profile picture is gone. All the things he was tagged in are now black and unclickable. I went on my friends account and still found the same thing (they were not friends). Is it a privacy setting or did he delete it? Or deactivate it?

It seems like he may of changed his privacy settings as if he had blocked you, you wouldn't even be able to see anything and it'll say you can't access the page.

sounds to me like his account was suspended by facebook
