Sometimes its connected to a twitter account

That someone put it there. As far as i know, it does not mean anything special, and facebook does not automatically put the # sign there.

# in front of a word does however mean something, and yes, it's connected to twitter.

The # is a hashtag. It's used to group tweets about a certain subject so they can later be searched for.

Otherwise, it could possibly be a browser issue, if it shows up on all of them. Sometimes browsers are stupid for a bit. Try changing to Firefox or Chrome. If it's still there, they put it there.
