Sometimes I am checking around who is on Facebook and who is not but recently it was my brother's ex-wife. I looked up and he asked me not to add her, so I won't. Will she know I had looked up her profile 2 times?

No, But facebook will. Facebook tracks all of your searches, So does yahoo, google, bing, etc.
So no, she will not know.

No she won't. Unless you do something that may let her know you have, e.g. likeing pics, status, or commenting. So don't worry.

No. Facebook doesn't even track profile visits themselves, there is no way to know if someone's looked at a profile or not.
