they can be using any program like photoshop, photofiltre, gimp...etc!
its just a backround and they make the code themselves(=

I use http://www.twitrbackgrounds.com for their free pre-made backgrounds.
I hear that http://www.twitbacks.com is pretty good for making your own free customized background.
