ive seen people use this on facebook and twitter... it seems to be some sort of commentary punctuation device but im not sure. thanks!

It's basically a tag for a certain thing. This thing can be anything you want, but if that tag gets popular it becomes a trending topic on twitter. You would basically use it to denote the subject of the post you make. So if you wanted to make a post about yahoo answers you would make a comment about it and the put #yahooanswers at the end.
