once you are at
facebook.com/home.php
on the left and side of your screen, you should see a left hand navigation that says "groups"
click that, then click
"+Create a New Group"

Go to the groups application
(old facebook: Select groups from the left sidebar)
(new facebook: move your mouse over applications and select groups).
Direct link: http://www.facebook.com/groups.php
Then click "Create group" (ie. http://www.facebook.com/groups/create.php )
