is there a form that you can use to try and active a suspended facebook account and is there a link where you can access this form

Why bother with all that hassle? Just signup a new e-mail address and create a new Facebook account by that address. It will take a heck of a lot less time then trying to go thought whatever dispute resolution processes they've setup.
