I saw the option "Ad Free Mail". What does this mean? Do they automatically install something when you click on this option?
Does the user have to pay something or not?

Well - you know GMail show ads above your mail, YMail is similar and most of other web mail system too.
Ad Free Mail - mean to use mail without show ads, nor contextual, nor banners.

Ad free email means an email which is free from all ads of the company who is offering them. It does not mean that you will not get add from those who have you have subscribed to or approved.
