The publisher puts it there although if you are self publishing on Amazon or Smashowrds then I don't know. Also you don't have to pay anything as paying that fee to the copyright office only allows you to sue for more if anyone ever plagiarizes you and you want to take them to court. If you are self publishing then the self publishing services don't do much of anything for you, you are supposed to do it yourself. If you go with a trade publisher no you don't put that on your manuscript when you submit ityou will just make them roll their eyes at your lack of knowledge.

There is no legal requirement to EVER have "copyright stuff" on your original works of creative authorship. Copyright is still there, whether or not you tell people it's there.
