She 'won' a contest and put her information on an online form including our house number and street, her phone number and other information. No card numbers though. I looked at the site/form and I'm sure that it is fake it was on (http://clixtk.com/). What could happen now is there anything I need to do? i'm 99% sure that it is fake and somebody has our living information is there anything that I should be worried about
On her computer the website is full of surveys which you 'win prizes' which is why she put the information in

That site is a relatively new site created on February 01-2013. It claims its "monetization solutions are the ultimate platform for content publishers, website owners, and online game & service developers. Generate profit from every visitor! Our Content Locker, Link Locker, & Product Locker tools easily protect premium content, upgrades, downloads, apps, guides & more."

My rule-of-the-thumb attitude to such sites is to be cautious particularly when it has no known online reputation. Secondly, your sister has no business filling any form on that site if she is not a content publisher online. this bring us to the third issue, safety/security of information divulged to that site. You cannot retrieve it once given. If she logs into the site with her login details (if an account was created for her) she may delete the information, otherwise, just exercise caution in filling your personal info next time on strange sites.
