Is the ONLY way for facebook to suggest a friend to me under the "suggested friends" list... if one of my friends physically suggests them to me by clicking on my name as a suggestion for them... or does Facebook have other ways of doing it, too?

No, it's not necessary that a friend has to physically suggest a friend so that it shows on suggested friend list. Facebook also looks at how many common friends do you have or any other parameters that facebook probably developed based on experience. For example if you and me visited the same college or have similar interests. But you still cant know for shore because that is added value of facebook, something like google's search matrix
