When a Third Party App is written for Facebook, say the developer has an idea for a game, etc... how do they do this? Do they need permission from Facebook, or work with them in some way, even peripherally? Thank you in advance for any responses.

Facebook is just another platform like iPhone, Android, Macintosh or Windows. You get the SDK from for the platform, write your application and publish it. Terms and conditions apply, but fundamentally Facebook don't really get involved.
