There is no any thumb rule for adding keywords on per page, it depends on your one page information. I will suggest you around 4 to 6 keywords should be added in one web page with high quality content information, then easily you can get top position in major search engines.

The keywords targeted per page should be used less in number in order to target them effectively. The best would be less than or equal to 3 keywords you can add for a single page of your website.

Well, it depends on page to page and keyword to keyword.

there is a direct relation between the content and the relevant keywords, i.e, whatever the content includes in the key phrases, keywords, you can target them all.

But, being practical and to actually bring the keywords on top of Google, you should start with 10 keywords in the starting and slowing increase 2-3 keywords every month.

You can add 3-4 keywords per page which is very closely related.
