Easy
1) Acquire cheap cellphone camera.
2) Take appallingly bad photograph
3) Upload .

Why would you want to?
It is as selfie that was shot using a wide angle lens which causes huge distortions of the persons face.
That and the lighting is just plain bad
Being out of focus and very noisy make this image forgettable.
If you spend some time learning how to compose your shots, they should be much better than what you linked to us

Do what fhotoace says:
take a picture of yourself using a wide angle lens in terrible lighting, make sure it is out of focus and with lots of noise.
And there you have it.
I'm sure you you add in a few additional effects like a little bit of red eye, some judicial cropping, some more stuff in the background etc
Time to learn. You'll enjoy it hopefully.

Why would you want to copy such a poor photograph!
There is no App involved. It was taken probably with a phone camera with bad lighting and a mediocre model.
