Is it visible to everyone who follows you, or just the person you are @replying too?

Depends if your followers are following that person as well. If they're not, than no. They can however, see the tweet if they were to look on your profile though. (:

Yes. They appear on the wall as typed. However a direct message is only seen by that person in their direct messages list. Twitter is a great tool for social networking.
