No, they actually know those people. I have a friend with over 2500 friends and she knows them all.

some people i know who have like 1000 friends know people from other schools and alot from our own

They meet people that aren't really their friends and add them

The way I reached over 1500 friends is by requesting new friends in each game that I was playing at the time. I eventually developed a huge list of friends. I know some of them in real life as well, be they relatives, people I knew growing up, or people I know now in real life. Mostly, though, the people are gaming buddies and some of them like to chat and are really nice once you get to know them.
