I uploaded a photo on Facebook about 3 weeks ago and tagged a friend of mine in it. Today, a person commented on that photo. Will my tagged friend receive a notification about that comment even if it's an old picture?

Yes, anyone tagged is susceptible to notifications, including the person who uploaded the image, whether or not they are tagged in it.
