I would like fans only to be able to fill a google form that is accessible from my fan page. How do I do it?... Do I need some code?

well you need to do is
1. go to google.com
2. click on google ads and it gives you a code
3.go to the edit of you fan page and go to edit
4.click enter codes
5.than do the Questions
6.and refresh

Good question. And yes you do need some code. There are ways to go about it manually but there are now some great tools to make it drag 'n drop easy.

Let me know how you go. You just need to register for free, follow the install instructions, and you will be able to put a google form on your fan page in a short matter of time without having to muck around with copying, pasting, code alteration, testing....
