You make an update with their name in it. For instance, if I wanted to send a message to Miley Cyrus (Twitter name=mileycyrus), I would say "@mileycyrus I love you!" or "I think @mileycyrus is a whore." As long as your update has the @ symbol followed by a Twitter username, they will see it. As for private messages... I don't know; I don't even use Twitter.
PS I don't love Miley Cyrus nor do I think she is a whore. My opinion of her remains strictly neutral and I used her only as an example.
