It seems that anyone, not just my friends, can make themselves my neighbor on Facebook games Cafeworld or Farmville. How do I prevent this from happening?

i don't know for sure everything that is going on on facebook, but I had the same problem.
when you receive the notification of an application, there has to be there a button that says block application or ignore.
and on the main wall, if a friend of yours says something about a game, and the post is made by the application itself, you can click on remove then it appears there something like: block (name of user) and block (name of application).
this is what i discovered so far. i hope it helps.
the same you do when in you inbox you have that sort of application invites or notifications. you can choose block application.

hey i'm sure they are your friends, because the only way to add neighbor is from accepting first
