I always see people use it but I don't know what it means. Please help.

A hashtag is used on twitter to track topics!
For example, there's an upcoming moving called "Take Out" you put on a "hashtag" / # to track that tweet.
It can go:
Just watched the movie #TakeOut it was incredible!

The hashtag is for grouping together all message talking about the same thing.
Instead of following everybody talking about it, you can "follow" a hashtag by searching it.

It's just a way to tag things.. Like pictures or stuff that you write.
For example; On Twitter people use hashtags to tag their text posts. So when people click on the tags, all the posts with that tag shows up.
