It means it costs you as much money to update twitter with your cell phone as it does to send your friend a text message.
So, that means that there's no additional charge to send a message to Twitter's 40404. Eg. if you have no messaging plan, most providers charge $0.25/message. But, if you have a plan, it just counts as just another text message without any extra fees.

You get charged for messages sent to twitter, and received from Twitter, just as you would for normal text messages. Every wireless provider is different, and have different available plans for included text messages.
