It is a Twitter hashtag, used so that people can keep their tweets in touch with others.
The 'HAN' is obviously a set of initials. The only way to find out would be to put '#HAN' into Twitter's search engine and see what comes up. But be warned, many different people could be using the hashtag to mean more than one thing.
