It's like tagging something for example:

It snowing Smile burr!!!! # cold

it means the number. normally "#" is known as the "number sign" whether people use the name hash on it

The symbol is called a "pound sign" because it is an abbreviation for pounds, as in how much something weighs. "I want to weigh 115 #, but I don't." But it's also used on Twitter to collect tweets about one particular thing in one place, so to speak. It's called a "hashtag" when someone uses it for this purpose. Say you tweet, "Great show last night! #americasgottalent." If other people also add that hashtag to their tweets about the show, and you want to see all of those tweets, you can search for that hashtag on Twitter and see what other people are saying about that super-adorable kid who tapped danced like a boss, or that Goth opera boy everyone is in love with (myself included, BTW.)

The same symbol appears on Facebook sometimes because it's possible to link your accounts to automatically cross-post between Twitter and Facebook, so if someone tweets something with a hashtag, the whole tweet will appear on their Facebook wall too.

I hope that helps!
