My Fb says that my friend became friends with someone. Is that because he confirmed a request or is it possibly because this other person confirmed his request?

Yes. When a Facebook friend add's/accepts a friend request, it now shows on a mutual friends News Feed,

You would be notified if your friend became friend of other.
i got your question:
it can't be said who send request to whom and who confirmed ...
but end result is they are friends.
