So on one of my friends Facebook profile it says that they are following 4 people yet it only displays 1, is there a reason for this?

The other three have been deleted or have chosen not to be followed.

Not to sure, their privacy settings could be set high?

They are hidden to public and friends. Facebook has settings to hide any of your activity.
Your friend just don't want others to know about those 3 people.
