I've never sent friend request to this person but it says this person accepted my friend request?

You could have sent one and forgotten, or it could be a mistake on Facebook's side. You can always delete them as a friend.

check your account for malicious apps

it's not a "mistake on facebook's side" - it's much more likely that you clicked on something malicious without realizing it

HACKKING or phishing..this can be possble reason as you have not sent request to that guyz..first of all change the password and do change your primary id as he may have control on it..wish you luck

Most Likely Someone Was On your Account and Sent Friend Requests to People i Would Change your password And Check your Apps For any Suspicious Apps

May be you forgotten but there is no need to worry.......... you can unfriend him at anytime !!
