I always thought it was when they were last active, but I have friends where it says 2d yet they have made status updates since. Also why would they keep disappearing and reappearing in the top section of chat bar?

The number indicates from how much time they are online.
When it shows mobile icon next to the names, they are online through mobile app.
And showing in chat bar is random thing. It shows random persons in the list but gives preference for those who are online...!
