You basically let your eyes go out of focus a bit and glance around the pic. These pictures are either specially crafted brain puzzles or just random **** that has something in it that you wouldn't expect to see, find humorous, find creepy, or say WTF when you see it.
Examples:
Man sitting in a corner with a murderous gaze during a groupshot of friends.
Some kind of glowing light in a dark area of a pic that look like demonic eyes.
An object in the picture resembles a face.
A topless/nude person randomly in the background.
