It means that the person that is following you will get notifications once you post something and such. It is almost the same as on Twitter. Let's say it is literally "following somebody".

Someone can follow yu because you turn on Follow in Follower under Audience Setting. Follower can only see public post and anything the person being followed do in public post, such as in Open Group, coments/likes in others' public post.
