If you want to follow 500'000 you need 505000 followers, they follow about 5000 every day,do the same

if someone is following that many people they either:

1) don't read every single tweet in their timeline
2) follow everyone who follows them back but they only really care about specific people's tweets so they make an effort to look at those people's tweets.

OR the logical answer is probably that they use lists. Lists are really helpful if you follow a lot of different sorts of people who tweet about different things. I have a bunch of lists for different sorts of people:

if I want to read about the news, I have a list of reporters and pundits. If I want to laugh, I have a list just for comedians. You can list pretty much anyone (even if you're not following them) it's a great alternative to following a bunch of people but if you *do* want to follow a lot of people, make sure you organize them into lists. That way when you're in a certain mood - let's say you want to know what the youtubers are up to at vidcon or something, you can go to your youtube list and see everyone's tweets.

The only limitation on this is if someone is a privated user and you they didn't accept a follow from you but you added them to a list, you won't be able to see their tweets.
