Log into your Twitter account. Click on your profile, then click on "Followers" in the top-left sidebar.

You will see a list of all your followers.

2
Scroll down until you find the follower you wish to remove. Click on the Profile icon to bring up a menu. From the list, select "Block @[username]"

3
Verify that have blocked the user. You will not receive a confirmation from Twitter that your block has been successful, but as soon as they're blocked you should see that the "Follow" button is now replaced by a "Blocked" button.

4
Unblock a previously-blocked user. You can unblock them at any time by scrolling over the "Blocked" button and clicking on it.

5
Understand blocking. Per Twitter, blocked users cannot:

Add your Twitter account to their lists.
Have their @replies or mentions show in your mentions tab (though those Tweets may appear in search)
Follow you.
See your profile picture on their profile page or in their timeline.
Note: if your tweets are public, they will still be visible on your profile page to anybody—including the blocked person.
They will receive no notice that you have blocked them.
