It means "number"
Such as,
I'm #1!
I'm number one!

in twitter is to indicate a subject such as #mets.
This makes it easier to trace the conversation because you can just search for #mets.
Without the # tag you can only read the tweets of people you following, thus with it you can read the whole conversation.
