I have two facebook accounts, because the first one got disabled. I have moved past it and don't care about my first account anymore, and want to delete it because it still shows up on my friends' friend list (which is kind of embarrassing) so I want to know how I can delete it for good. Remember that I can't log in.

The only way I know how to find this form is by logging into the acct you want permanently deleted. Then in the upper right hand corner click that little wheel that gives you acct and privacy settings. Skip over those and go to the help button click on it. Type in permanently delete my acct. It will show you the question. click on it. That will give you the form you need. You might not see it at first. It is a highlighted link that says fill out this form. Click on it. This will permantely delete your acct rather than just deactivate it. It will immediately disappear from facebook. But you will have 14 days to change your mind. If you change your mind all you have to do is login and it will give the the option to open it back up and continue to use it.

If you can't even log in to the "disabled" account, all you do is contact facebook and have them help you get the old account enabled again so you can either wipe the information off of your wall, or Permanently Delete that old facebook account. This may be quite a lengthy process to get facebook to answer you due to the size of their customer base. Contact facebook via your current account.
