It's called a hash tag. People use a hash tag next to name or phrase to make it popular. Often you will hear about "trending topics" - if you searched Twitter for David Beckham for example you would find every tweet that has #DavidBeckham in it.

Basically it is just used to represent a popular phrase, person, or place.
