Yeah, it sounds like it's best to tell the doctor everything you dare speak about. At the very least it will get it off your chest, and that'll reduce some of the stress.

Just a tip: the doctor's only human. So I suggest that you make a list of single-line (less than twitter-length) notes on your computer and then, after you've got a dozen or so, sort them by priority, and group similar items together. Keep typing notes and sorting/grouping them until you're out of ideas about things to write down.

Why this extra effort? This way you'll be able to tell the doc the maximum amount of information in the minimum time, which means he or she will have a better idea of what's up. If you just say stuff at random you make his or her job much more difficult. The quicker he or she gets the big picture the more time he or she has to work with you on the issues.
