It's a feature in Twitter.
What it does is to make that post become easier accessible by searching for the specific groups or words on twitter that's written after the hashtags.
Sometimes you might see this on Facebook as well but that's because most Twitter users have their Facebook account and Twitter accounts synced so when they write something on Twitter it automatically transfers to Facebook including the hashtags.
