I need two different types of model release form?

You only need one type of model release, but you can modify the release depending on how you wish to use the photos. If a company is asking you to shoot for them, find out if they have a model release, and how much they plan on paying the models.

You basically want the release to say that the model grants you and the company full and unrevokable commercial and non-commercial rights to post any and all photos, including edited photo, in any print or electronic media for now and all time in exchange for the valuable consideration of (whatever they are paid).

Do an internet search for a model release and use whichever one you like. I have an app on my phone that allows people to sign electronically, but you need to have a contract already prepared.

For your facebook page, you can tone the language down unless you think you are gong to be selling the photos.
