Hardly ever to experience such things to happen. But then, there are some prominent factors to be considered to stay away from the inefficiencies to happen.

From my outlook, it could be of
- Product
- Service

May a be a few of about 10% could be due to the information that keeps us to fall behind.
If those two criteria are filled to the required, there will not be any such unskillfulness to happen.
