Larry your doing just fine my friend, everything is there contact form, address, phone, ceo up front and center shows trust. You have everything you need to generate leads on this page.
The trouble is you have more then this page needs to sell. For starters remove the product images shrink everything up above the fold so visitors can see everything they need when the site loads. Try your unique selling proposition in an H1 tag on top of everything. Address, contact and your image to the left with maybe a link to your bio on site. Your contact form to the right with a field that drops down for what appliance they need repaired. To help the forms conversion only ask for the information you require to finalize the sale the less fields the better. Maybe a nice bright yellow submit button to make it POP! Instead of all that navigation on top put your contact phone number nice and large. Bring generating lead to the highest priority of the design. Need any help find our link in the source box.
