It's called a hash tag. It was made popular on Twitter as a way to find people who are referring to a common topic. You're likely seeing it on Facebook as people you know have linked their Twitter and FaceBook status updates.

How does it work? Let's say someone makes the status update: "Yahoo Answers is really useful #yahooanswers". Someone could then search for #yahooanswers to see what other people are saying about the same topic.

Twitter uses the very same hash tag to see what topics are "trending" -- that is, what's currently popular.
