When logging in to facebook has anyone been asked to fill out a 4 question survey?

I recently logged on to facebook and a page came up saying that facebook was doing a survey. It asked 4 questions about the age of the user, if facebook was the only social site used by the user, how many friends onpreferredd social site. It then said that there was a gift for answering the survey. Has this happened to anyone else? Is this a ruse to get me to put in my e-mail address and home address so they can send me advertisements?
I didnt fill it out, I was just curious if anyone else came across this

That is a fake fb website. if u access through some links this will happen often.
