When i click on new notification on facebook, it sometimes shows the older notification again. Why ?

When i click on new notification on facebook, it sometimes shows the older notification again. Why ?
nowadays when i click on a new notification or a comment received on Facebook it many times shows me one particular earlier comment in the list and does not the display the new notification at all. does anybody know the reason behind this ?
