Go to account click on privacy settings then scroll down the page you will see something that says Applications and Websites then click on the edit your settings which is right under it,then at the top of the page you will see something that says ''Applications you use'' then click the edit settings button,and then once you see the app you want to delete click the x button and then it will ask you if you want to delete it or not then click remove.
