My Friend Didn't get the friend request on facebook?

okay so there's this girl in my leadership class that i know & talk to
alot & we're become really good friends .
And so she asked me to add her on facebook & i said sure .
So after school that day i logged onto facebook & typed in her name & she had 52 mutual friends . & that was obvious that was her .
So i went to her profile & clicked " add as a friend" .
Then the next day she said that she didn't get the request . So i went back on facebook & checked . It still said " Friend request sent " . Why didn't she get my request ? It's been like a week now .
