HOW DO I REMOVE THIS PLUGIN FOR FACEBOOK LAYOUTS ?

right i went to this website http://freecodesource.com/facebook-layouts/
and installed a plugin that will help me put layouts for my facebook .
i then found out there are advertisements , and are quite annoying , so theres a disable button on top left of my facebook page .
but then , the advertisements remain !!!
its not the facebook advertisements , but the site's advertisements !!
HOW DO I REMOVE IT ???
im using google chrome btw

first look under Control Panel, add/remove programs and see if there is any entry there that looks a likely candidate. If so, uninstall.
If there is nothing showing there, download and run the free anti-malware app Superantispyware, which should clean things up for you.
If you want to be extra sure, then also run Malwarebytes.

Go to Control Panel And Or Options Depending on which browser you are using may be worded differently than once there go to add ons than find the add on than click disable or remove and it should be gone once you complete that

At the upper right corner of your Chrome browser, there is a small wrench button, click the wrench, and click Option in the menu, then "under the hood" tab, "content settings" click "plug-ins" and you can modify from there all the plug-ins you have added in your chrome.

But if you want to completely delete it in your machine, then follow the previous answers above.
