Know who your friends are, they say. = Know who means you well; know who you can trust to have your best interests at heart.
I don't think I could trust anyone from where I live to be frank. = I don't think I could trust anyone from where I live to be honest and outspoken with me. OR
I don't think I could trust anyone from where I live, to be frank. = To be honest, I don't think I have any friends where I live.
