Girls why did she add me on Facebook?

This girl I work with asked me today why I never accepted her friend request on FB, I told her I never got it, asked her multiple times and she swears she sent one, so I sent it from my phone and she added me, but if she wanted me to add her, why didn't just ask if I had FB and to add her?

Thing is I'm friends with 6 of the 8 other ppl I work with on FB, some of whom she talks to more than me, and she isn't friends with any of them, so I'm confused, cuz why would she have sent me a request in the first place, since we don't talk much anyway, and she has a bf. I'm confused.
