how do you start one of those group/fan accounts on facebook?

You click on a Pre-Existing Fan page and int the Top right hand corner there should be a tab that says (Create Page +) and you click on that and the rest it explains itself

You go on the Facebook homepage not logged it and right under Sign Up there is a Create a Page link, click that. It'll ask you a few questions and once you're done you'll have a page.
