Whenever there is a change to a Facebook account, Facebook notifies the person via the email they use to sign in with. Check that email and follow the instructions you are given where it says "If this wasn't me then...." part.

Once that is taken care of, change your password to something no one will be able to guess at and never leave your Facebook open when you aren't present. Most "hackings" are done by friends and family who know what password a person may choose, or someone in the home when Facebook is open on the computer. Same applies to your phone.
